How can I request a printed copy of my Form 1095‑C if I did not elect to receive it electronically?

Employees may request to receive a printed copy of their Form 1095‑C. To request a paper copy, please contact the Benefits Office by email at Benefits@cherokeek12.net or by phone at 770‑479‑1871. Please include your full name, employee ID (if available), and your current mailing address. Your printed Form 1095‑C will be mailed to you within 30 days of receiving your request, as permitted under IRS guidelines.
