School Nurses

All CCSD schools have a school nurse to serve as a health resource and liaison, dispense medication, and render short-term care for injuries and illness.  All CCSD nurses are Registered Nurses (RN) or Licensed Practical Nurses (LPN).

Access Health Forms

Health Forms (medical authorization form, authorization to carry inhaler/epi pen/diabetic equipment, lice control, reporting communicable diseases and state-required immunizations)

Please contact the Lead Nurse for your school zone if your child requires management during the school day for one of the following health conditions:

  • Allergies
  • Asthma
  • Diabetes
  • Feeding Orders
  • Gastrostomy Tubes
  • Intermittent Catheterizations
  • Intravenous Medication
  • Mechanical Ventilation
  • Oxygen Therapy
  • Seizures
  • Suctioning
  • Tracheostomy
